| BUE | SNF Upper Body Exercise Program (OT-Focused) 1️⃣ Seated Shoulder Flexion / Abduction Purpose: Maintain/improve ROM, postural control, reach for ADLs Setup: Seated at edge of bed or chair Exercise: Pt raises arms forward (flexion) or sideways (abduction) to comfortable range. Therapist provides hands-on assistance or verbal cues as needed. 8–12 reps, 2–3 sets, slow controlled motion. Skilled Justification: Pt requires skilled facilitation to maintain midline and reduce compensatory trunk movement. Functional carryover: Reaching for grooming, dressing, or high shelves. 2️⃣ Scapular Retraction / Squeeze Purpose: Improve scapular stability, posture, and functional reach Setup: Seated, elbows bent at sides Exercise: Pt squeezes shoulder blades together and holds 3–5 sec. Therapist provides verbal or tactile cues. 8–10 reps, 2 sets. Functional Relevance: Reaching behind for hygiene, dressing, transfers. Skilled Justification: Skilled cues needed due to poor postural awareness and limited motor control. 3️⃣ Biceps / Elbow Flexion Purpose: Strengthen functional lifting, carrying, and grooming tasks Setup: Seated, with or without light hand weights or resistance bands Exercise: Pt bends elbows to bring hands toward shoulders. Therapist provides manual guidance or resistance as needed. 8–12 reps, 2–3 sets. Skilled Justification: Pt requires hands-on facilitation for controlled motion. Prevents compensatory shoulder shrug or trunk lean. 4️⃣ Triceps / Elbow Extension Purpose: Functional pushing, transfer, and dressing activities Setup: Seated or standing Exercise: Pt extends elbows behind body or over head. Therapist assists to ensure correct range and safe form. 8–12 reps, 2 sets. Functional Relevance: Reaching for overhead items, pushing off armrests. 5️⃣ Wrist / Hand Exercises Purpose: Maintain dexterity, grasp, and pinch strength Exercises: Wrist flexion / extension: Use light weights or no weight, 10 reps Thumb opposition: Touch each fingertip, 5–10 reps Squeeze therapy ball: 10 reps, 2–3 sets Skilled Justification: Pt requires cueing for correct positioning and coordinated movement. Functional carryover: Eating, grooming, writing, buttoning. 6️⃣ Shoulder External / Internal Rotation Purpose: Functional rotation for dressing, toileting, and grooming Setup: Seated, elbows at sides, 90° flexion Exercise: Rotate forearm outward (external) and inward (internal) slowly. Therapist provides manual guidance or cues for smooth motion. 8–10 reps, 2 sets. Skilled Justification: Pt demonstrates limited motor control or cognitive impairments, requiring therapist cueing. 7️⃣ Pulley / Overhead Reach Purpose: ROM, shoulder flexion, functional reach Setup: Overhead pulley system or towel over rod Exercise: Pt pulls pulley with one or both arms to raise hands overhead Therapist assists and monitors for safe range 8–12 reps Functional Relevance: Reaching for overhead cabinets, grooming, transfers Progression / Safety Tips Start seated and supported, progress to standing if tolerated Use light weights or bands as tolerated Emphasize slow, controlled motion and proper posture Frequent verbal, visual, or tactile cues for dementia, COPD, or cognitive impairment Monitor SOB, fatigue, and pain Denial-Proof Skilled Language Skilled OT required to facilitate safe, controlled upper body exercises due to impaired motor planning, postural instability, and cognitive deficits.
